Your first visit will usually consist of a full oral exam and consultation with your oral surgeon. This initial appointment gives your surgeon the opportunity to discuss your situation and treatment options with you thoroughly before forming your treatment plan. If you have any questions about your condition and treatment options, your initial consultation is a good opportunity to talk to your oral surgeon. If you need to have additional X-rays or 3D scans taken, your surgeon may take these during the consultation.
To help us provide you with care as efficiently as possible, we ask that you please bring the following things to your first appointment:
- Your health and/or dental insurance information (if any)
- A list of any current medications you are taking
- Any referral forms or X-rays given to you by your dentist or general physician
- A valid driver’s license or photo ID
- Your payment information (credit card number, checkbook, etc.)
Patients who are under the age of 18 must be accompanied by a parent or legal guardian at all appointments.
We recommend coming in a few minutes early so that you can fill out any necessary paperwork about your medical history or insurance information. If you have any existing medical conditions that your doctor should know about, please be sure to discuss them during your consultation. Feel free to contact our offices if you have any questions or to schedule your initial appointment with Dr. Yamamoto or Dr. Kitamura.
Thank you for taking the time to review this important information, and we look forward to meeting you.
